Appointment Policies
Scheduling
Appointments can be scheduled by phone at (615) 907-3409 or through our online scheduling system when available. Confirmation of your appointment constitutes agreement to these terms.
Cancellation Policy
We request at least 24 hours notice for appointment cancellations or rescheduling. This allows us to offer the appointment time to other patients. Repeated no-shows or late cancellations may result in:
- A missed appointment fee
- Requirement to prepay for future appointments
- Possible dismissal from the practice
Late Arrivals
Please arrive on time for your scheduled appointment. If you arrive more than 15 minutes late, we may need to reschedule your appointment to ensure adequate time for treatment and to avoid delays for other patients.

We make every effort to accommodate dental emergencies on the same day. Emergency patients may experience wait times, and treatment may be limited to addressing the immediate concern.
Payment and Insurance
Payment Responsibility
You are responsible for payment of all services rendered. Payment is due at the time of service unless other arrangements have been made in advance.
Insurance
We accept most major dental insurance plans and will file claims on your behalf as a courtesy. However, your insurance policy is a contract between you and your insurance company. You are responsible for:

Payment Plans

Collections
Accounts that remain unpaid may be sent to a collection agency. You will be responsible for any collection costs, including attorney fees, incurred in collecting your balance.
Patient Responsibilities
